Physicians may expect to encounter a greater number of questions regarding end-of-life care and living wills, and wish to have resources available to which they can refer their patients. The website www.uslivingwillregistry.com/forms.shtm has a number of links to various sites, including the American Bar Association, that offer information about preparing limited powers of attorney and advance directives. The " Five Wishes " document, seen on the " Today Show " and mentioned in this week's Time magazine, is currently legal for use in 36 states, and may be purchased for a nominal fee at www.agingwithdignity.org/5wishes.html.
