Minimally invasive spine surgery (MISS) involves, among other things, the use of endoscopes and lasers. It avoids the large incision and extensive manipulation of muscles and nerves common with traditional spine surgery. Some procedures might even be done on an outpatient basis, believe it or not. I also recommend that you read a couple of my prior posting, numbers 1587 and 1591. The New Yorker article, as I mentioned, is on Dr. Chiu's old web page. As far as I can tell, Dr. Chiu's California Spine Center is the most modern, most fully equipped anywhere. If you click on the sidebars to the left of the page you can access presentations and additional information concerning minimally invasive spine surgery. There are MISS practitioners around the country, but beware of unethical, poorly trained surgeons who might be in it for quick bucks. As yet MISS (being still - relatively - in its infancy) isn't a medically recognized speciality, however an ethical MISS surgeon will be well trained in the techniques and will have all the special tools. Here is an excerpt from Lynda's prior message. " If surgery is an option, see if Endospopic is an option for you, recovery time is less and pain from the surgery should be significantly less than if they have to do an incision. " It is " minimally invasive spine SURGERY. " Fusion, by its very nature, is massively invasive and for that reason any ethical practioner (in my opinion) ought to rank fusion as the very last resort in his treatment algorithm. The FIRST option for every conscientious spine doctor, again in my opinion, should be conservative management. Unless absolutely contraindicated, the physician might first try a 12 week program of exercise coupled with other conservative modalities. If that fails, he would then proceed to the next option. Of course there could be rare instances when fusion would be the only option - fractures and tumors for instance. Fusion is outrageously lucrative for the surgeon, for the instrument maker, and for the hospital. One physician has called the pedicle screw instrumentation " expensive tinker toys. " My good friend, a cracker-jack spine man, devotes a good part of his practice to removing failed instruments. Of course for the patient the recovery after removal is problematic, long and difficult. So I am just taking it easy right now sleep is next to nothing these days and I have a TON of children at my house constantly so there is no time to take a break. I made a decision today that i will not have another spinal fusion. If there is nothing else that can be done--than my Pain mahagement doctor will have to help me deal with it--unless im crawling on my knees---and then try not to still have it.
