Guest guest Posted November 17, 2008 Report Share Posted November 17, 2008 Hi, it's been awhile since I've posted, but I have another medicine question. went to the ped rheumy on Thursday and yay, he shows no signs of active arthritis. She feels it is because of the Clinoril and wants to keep him on it for 18-24 months before she starts to taper him off of the Clinoril. However, the Clinoril is causing severe stomach pain. They doubled his dose of Prevacid from 15mg once daily to 15mg twice daily in August. This seemed to help a little, but never completely took away the stomach pain. He recently learned how to swallow pills, so the ped rheumy added Cytotec to try to protect his stomach. My question is how long does the Cytotec take to start working. I had to pick him up early from school again today because of his stomach. This is the 11th day of school he has missed since we started after Labor Day. I just wonder if this is a medicine that takes awhile to start working or if we should see results quite promptly. The rheumy said if this doesn't work she will send him to see a gastro doc. I was going to suggested an anti nausea med because that is what Grant takes but he must not have nausea? It is only stomach pains? I think that being sent to a gastro doctor (and I am certainly no expert) seems silly because you know what is causing the pain and by stopping the clinoril would stop the pains! I think that the ped rheumy should be trying to find a different med that will still work for the arthritis and not cause the stomach pain. may be free of active arthritis, which is very exciting, but if he is still missing school because of pain caused by the medication then I would say that the medication isn't working and another one, that isn't an NSAID should be tried. Like Methotrexate? In fact it was NSAIDs that caused Hannah's stomach ulcers and she can now no longer take them ever again. The only way we found this out was by a ped GI. She is now on Tylenol and will need to take Nexium twice per day for a minimum of 6 more months and then will be on once per day for a very long time. With the pain that is severe or basically continuous it might need to be checked by a ped GI just to rule out the possibility ulcers have formed from the meds. Ulcers really need to be watched. had stomach pain from his NSAID, but once we switched meds it stopped. Just my 2 cents! Guest guest Posted November 18, 2008 Report Share Posted November 18, 2008 Thanks everyone. His ped rheumy has switched his meds 3 times, but every time it is just another NSAID. She won't start him on metho or any other meds since he is being controlled on the Clinoril. The last 2 days the Cytotec seems to actually be making the stomach pain worse. I was under the impression that it was supposed to increase the mucous lining in the stomach and help ease the pain. From all I have learned about JIA, NSAIDS do not stop the progression of the disease. Nsaids stop pain and inflammation. I do believe there are some that can control their JRA with just Nsaids but the majority can not. It sounds like has it pretty bad too when he can't walk after 4 days of being off the Nsaids. I would call your ped rheumy and question her more on starting a Dmard like MTX. If he cannot go off of them without pain, he is not really well controlled, at least according to my son's ped rheumys (he had two). The goal was to get him off the NSAID's and be only on a DMARD; mtx ended up working best for him. Chris' rheumy really wanted him off the Celebrex he was on as he believed the side effects were worse. Of course, this is how my son's dr felt. I do agree with that talking with the dr about adding a drug that will attack the disease rather than just help with the symptoms might be the next course. Plus from what I have read lately, the newest approach to the disease is for drs to be more aggressive early on in the diagnosis, adding DMARD's sooner rather than later as they did in the past. Guest guest Posted November 18, 2008 Report Share Posted November 18, 2008 - just a quick thought after reading this thread, I think your instinct is right that something is not right with having such stomach pain, and you are right to question and make sure that you and the dr are in agreement as to the best thing for . Getting him pain free is always the goal, as I was told from the very first visit with my son's ped rheumy. Whether that pain is from the disease, or from the meds used to treat it, pain is an indicator something is not right. As always, this list is here to throw ideas out there and offer support if you need it. Hope some of these thoughts help.
