Guest guest Posted January 4, 2007 Report Share Posted January 4, 2007 Becki- My daughter took mtx from the time she was 2 until around age 4 (at 7 she is back on it, but for iritis). She has pauci also. However, NSAIDs were not controlling it and she went quickly from one joint affected to 4, so mtx was deemed appropriate. She has never had any problems with it at all and it put her into a remission which continues today. Hope this helps. My daughter is almost 2 and has been on methotrexate for about 10 months. It took about 3 months for it to start working, and she did well on it for a while, but is now back to having swollen joints, rashes and fevers-she's systemic. She's had no bad side effects from the mtx-just a little less hungry the day after her shot, and maybe some slight hair loss. Our daughter is still on prednisone-has been since onset, so we are also considering Enbrel in addition to the mtx to get her off the pred. So I'm wondering too-anyone have experience with a little one on Enbrel, and how did it work? any systemics have good experiences? (Are there any biologics approved for a 2 year old?) And just because I'm full of questions today, do joint injections tend to work better with Enbrel, or about the same as without? Unfortunately, she is not in your daughter's age range, but I can give you our experiences with enbrel. n is 17 and came down with systemic JRA at age 10. She is better on the combination of enbrel/mtx, than she was on mtx alone. We were getting joint injections every 3 months when she was only on mtx. When enbrel was added she would go about a year at a time between injections. I can't say if they worked better, or if the enbrel just controlled her arthritis better. n isn't perfect with her current medications. I would like her to move to kineret, but I can't talk her into daily injections. She is not on prednisone. It is very rare that a systemic is controlled with mtx only, or least that has been my experience. Good luck. Guest guest Posted January 4, 2007 Report Share Posted January 4, 2007 Hi Becky, Becki here.My son is systemic,so not pauci either. was diagnosed a month before his 3rd birthday.By the time he was 4 he was maxed out on 25mg sub q MTX,200mg Plaquenil and 300mg Ibuprofen 3x a day plus the pred daily between 30mg to 6mg.Two ankle injections within 3 months,and at the ripe old age of 4yrs and 2months they added .45mg Enbrel 2x weekly. Because of the constant use of steroids for 21 months also weighed 62.4 lbs when he started Enbrel.All of Abby's doses would be much lower. Don't let Enbrel's FDA thing about 4 and up scare you.That was the youngest age allowed in the original drug trials back in 2000. I hope Abby does well with whatever you decide to try. My daughter is 4 and she has been on mtx since she was 2 and then Enbrel was added when she was 3. She is poly and Enbrel has been a miracle drug for her. The NSAIDs just didn't do it for her nor the mtx alone so the Enbrel was added and it has been great! They now say she is in medicated remission and we have been able to cut back very small amount on the mtx. If things continue the same when we go back in March, they will cut back some more. Kaylee has had almost no side affects from mtx or Enbrel. When she could not hardly move anymore she had steriod injections. That did wonders for her. When she was 3yr. The mtx was not working so well for her and the doctor kept uping the med., to the point she was at a max. what a child could take. We had to take her off the mtx because her liver was getting bad and she was turning yellow. So at 4 yr. she was put on enbrel. It has done wonders for her. Hardly no flair ups anymore.The only problem we have now is the iritis which came on after she was taken off the mtx. My dauughter will me 8 in march.
