Question About NSAIDs

This is probably a very common problem for most, but I thought I'd ask if anyone

has suggestions for a child who just can't tolerate NSAIDs on a daily basis. My

son, 7 years old, has had JRA since he was 3. We've done the Aristospan

injections, which seem to help for 9-12 months. But he is unable to tolerate

daily NSAIDs. They really upset his stomach and nothing seems to help.

Does anyone have any suggestions?

Holly uses the flavors on the prednisolone, she picked the sour apple i

think it is.......she says it makes a difference, and pred is so awful to

cover!!! How is bran doing??

Bran takes spells. He still has tummy problems and chest hurting but better.

We really think there may be some school anxiety and not sure how to handle

it. Any ideas??

Donna,

I wish I knew about the flavors back in the pred days.

s sister Shelby immediately threw up the first dose of Zithromax she

ever took.

Later we had it flavored cherry even though it was already cherry but it was

much more tolerable.

Luckily both kids are old enough for a z pack.

Doesn't it already come peppermint? That's what Caroline's and Evan's

are...

Hey Donna,

Robbie has been taking Zantac for years - I do believe it's a mint flavor (do

they know that most 4-5 year olds don't like mint?) - that's how old Robbie was

when he began the Zantac. Almost all of Robbie's meds are still in liquid form.

The Cyclosporine is the " skinny yucky one " the Zantac is the " fat yucky one " and

the pred is the " red yucky one " - though he complains less about the taste of

the pred than any of the others - I guess he is just used to taking the pred.

The skinny and fat refer to the size of the syringe I draw them up in - he does

better if I wipe the outside of the syringe off so that none of the meds get on

his mouth, when he squirts the syringe into his mouth. He used to drink a small

amount of milk as a " chaser " - but hasn't required the milk now for a few months

- when he is getting Motrin too, he uses that as a chaser. It does help that

his folic acid and lisinopril pills are small enough to take in a small spoonful

of pudding (chocolate) - which he takes after his other meds - well, almost

last, that is followed by the Viactive calcium chew, also chocolate flavored.

When he was in the hospital once, the Staff Pediatrician was insisting that he

learn to swallow the pred capsule - and Robbie kept choking on it, spitting it

up (it had little tiny balls in the capsule - at one point I had the pudding we

attempted to get the capsule down with - with all the little tiny circles from

the capsule all over me. The Doctor said " Well, he is just going to have to

learn to swallow it! " He was so hateful, and I told him there was no way in %$#

I was putting my son through that every morning - give him the liquid - he kept

arguing with me that all the kids hate the taste, they always throw up with it,

and that he would never get it down. I told him to go take a look at Robbie's

record, that he had been taking this for 2 years (at that time) in liquid form

- he then tried to tell me it was a much higher dose - I told him then he'd been

at higher doses. I finally won -but what a battle!

Aww.... That would be part of the problem then. I think the Zantac is

naturally minty so I am not sure what you can flavor it with to mask

that... I would ask the pharmacist - they are usually really good about

that kind of stuff..

We are good here. Evan has a little cold, so we are just holding our

breath that Caroline doesn't get it. Her last two birthday's have been

terrible (last year Pneumonia and then Influenza). We are hoping this

will be her year to finally feel good!

kill the taste (i.e. take the Zantac and then immediately bite into a

piece of chocolate). I have heard this sometimes works..

That is ridcuslous if Robbie wants liquid give him liquid. They did that to me

when I was little, they told my mom that since I had to take medicine all the

time I needed to learn to swollow pills, so I was forced to, I learned at about

age 3 to swollow my pills, we practiced with tic tacs.. it was so ridculous

though there was no reason for me to have to get pill form of everything at 3.

Doctors need to realize its so uncomfortable being a kid with a disease and if

having a liquid med makes it easier then let the kid have it!

Hi ! Baby Evan is getting so big! He's almost 7 months old now and

weighs about 16 pounds (only 10th percentile!). He is starting to scoot

a little - I am sure he'll be crawling in no time. His reflux is still

an issue, so we are probably switching meds this week. I have to call

his pediatrician and see what's up.

Caroline is getting excited for her big day. I just can't believe she is

going to be 4! It's crazy! She is doing pretty good. She has the tiniest

of flares going on with her knee's, but it's nothing a little Tylenol

isn't helping. She's been illness free for about the past 6 weeks so we

are doing a mini celebration dance here. If she can make it to her March

appointment without getting sick we get to start Remicade and get rid of

some of these meds!!

How are you doing?? Have you heard back from the other two colleges

yet??

I agree with you . I think that the underlying issue there is the medical

community failing to REALLY listen - and communicate with the parents of the

children, when the child is the patient. Not always a problem, but

sometimes....So much could have been avoided had he said " this is a much higher

dose of the pred thn your son has been taking, so we thought we should try the

capsules... " - that could have led to a hopefully productive conversation which

would have had the same, though less traumatic and stressful path towards the

end, where they gave him the liquid med after all. I keep thinking that I

should start working with Robbie and swallowing pills - starting with tic tacs

or M & Ms - but he takes so many meds - as long as he wants the pudding to get the

little pills down, he can have it. I never have a problem with him taking his

medication - for years now he has gone to the kitchen counter after breakfast,

or before bedtime, where the syringes & spoon are lined up waiting for him, and

just gives them to himself. He used to tell me when I had them out of order (I

can't remember if he wants the zantac or cyclosporine to be first in line) - but

now he just corrects the order and takes his meds. I hope that you are feeling

better!

Well said Val... We are lucky with Caroline's rheumy. She doesn't think

we should even attempt pills until Caroline is 5 or 6.... It does make

it harder to find some medicines, but since Caroline can't take any

NSAID's we haven't run into too many binds...

Well, I see you've gotten quite a few responses, but

I've only had a few minutes to skim through them. I

thought I'd add my 2 cents, but please forgive me if I

repeat anything anyone else has said!

Have you mentioned the problem to your son's pediatric

rheumatologist? My daughter always had problems with

her stomach tolerating NSAIDs, and her ped. rheum. was

sensitive to that. She used to take a daily tums, and

now she takes a daily Pepcid Complete.

There was a time when things were really bad (two

years ago). She was on Sulfasalazine for a month

until we realized it was causing major problems with

her digestive system, and her esophagus needed some

time to heal, so liquid Zantac was prescribed for her.

As with most kids, she hated the taste. In fact,

when I picked it up from the pharmacy, the pharmacist

came out to me and told me that the taste is simply

awful, and that some kids found that if mixed with

equal parts Hershey's syrup, it was a bit more

palatable. My husband worked each night to fill that

oral syringe with half Zantac and half chocolate

syrup. We got through those 10 weeks of taking

Zantac, but my daughter was so relieved to be able to

start taking a Pepcid Complete tablet after that!

Hi,

Instead of zantac. We did Famotidine or pepcid 10 mg over the counter once a

day which is available in chewable tablets flavored and it seemed to work great

for Joe- he is 4 y/o and around 20 kg. We tried the liquid stuff and it was

gross and expensive.

I did my research and famotidine has been used in kids and is just as

efficacious as ranitidine/zantac. So I got the ped rheum OK to give pepcid

instead and I could not find a chewable Zantac ect. - and she was fine with it.
