If the school has a Elevator which 's Middle School did request for a Elevator pass. This will cut down on time as well as strain of the steps. Also add this just in case. Help with lunch tray. This can be done also by talking with the Cafe' Manager. Be sure to take pamplets with you enough for all teacher's including the P.E teacher. Another to add is P.E. most school district's have a Physical Therapist on staff. Since most of the JRA kids need PT during the year this will come in handy. This part only takes a note from the doctor. If you want take a flyer about JRA and ask that it be displayed in the school in different areas. Since theres a child in the school with it. Another thing is to be informed of any testing done. This way it's good to make sure the child has enough rest prior to testing day. Pencils and Pens lolol. You didn't think of this one I bet. See if the school has big pencils and pens. This helped with gripping and writing. If the school don't get a supply of these. It's best to add more in a 504 plan than not enough. Try covering as much as you can think of. This way if you need it it will already be in place. Now if writing becomes a problem. > The school district has funds for adjustments for writing. You request it. The federal goverment funds laptops for kids who need them to help. This sure has helped with her school work. Gripping a pencil is alot harder than typing. Typing will exercise the fingers also. Last but not least in the 504 plan. Ask that a student be assigned to help your child if needed. ok lets go over it 1, Extra time for work 2. Break time during class to stand and move around. 3. Extra time for getting to class 4. Extra time for bathroom breaks 5. Medication to be carried on Person 6. Assigned Student helper 7. Extra time on ALL Test's 8. Laptop Computer if possible 9. Elevator Pass 10. Extra set of book's to be at home 11. Big Pencil's and Pen's 12. P.E. Accomadation's ( P.T if possible) 13. Tardiness acceptance due to Medical reason 14. Team work between ALL Teacher's and Parent in the best interest of the child. Give out Email addresses and phone number's. You want to be informed of everything concerning your child as well as you informing the Teacher's. 15. You have already gotten great advice, which we can use to, so thanks for asking this question. At 's junior high you get 15 passes per quarter. these are for being tardy, needing to use the restroom during class, forgetting supplies, essentially these are used for everything. had to play with her meds this year due to Vioxx being removed and then liver function problems. For 3 weeks she tried Voltaren XR and this would give her sudden stomach pains and diarrhea. She got permission to just leave the classroom if her med was making her sick, without having to ask the teacher and get her book signed first. If Holly's school gives limited number of passes, maybe she can have an unlimited number. Another thing that we had to have in place was that she was not allowed to be penalized for what she could not do in phy ed class. You would think that would be a no brainer if you have a kid with JRA, but for her first phy ed teacher in 7th grade it was not. He gave her a B first semester and told me directly she would get a higher grade if she ran the mile faster, it was at that point we got her school involved. Sad to say, but you just never know what kind of teachers you are going to have. Good luck putting together the 504, and I hope Holly is feeling better. Hannah's 504 includes all of her health issues, not just her joint issues. How soon do you need it. I don't have a way to scan it to email it to you, I would have to mail you a hard copy. For my teaching practicum class we will be discussing students with special needs and as a student who had a special need (boo arthritis) I thought it would be pertinent to bring in information about pediatric arthritis and the modifications children require. I think it's such a cool thing to be able to show these future educators what they need to do in order to help students who have arthritis in their class. I figured it would be a great way to raise awareness and a chance for me to share my own experience of being on the other end (because it definitely has impacted me as an educator). Unfortunately I cannot find a copy of my 504 from elementary, middle or high school. I will not use your child's name if you do allow me to use your 504, more so I will discuss what the dx is and what modifications are placed because of it. Of course if you wanted me to use the name I'd be ok with that too. Please help if you can. Thanks a bunch.
