Hi Jim,

I can only say hopefully your neuro will know what to look for as far as

changes and find something they missed. Have you noticed any improvements

with ? I do hope so, you have fought for this and are opening doors for

many of us parents. Please let me know what happens.

Part of the approval process for my son to receive a second set

of hbot treatments from amerigroup new jersey was to have pre and

post spect scans performed during the first session. We did this.

The radiologist transcribed on the report that kyle had a normal

brain perfusion spect tomography and both spect scans appear to be

normal/identical. I spoke w/ the radiologist and he said had no

decreased blood flow. Does this sound possible? the technique used

was 10 mCi of 99m/Tc Neurolite Radiopharmaceutical. Any suggestions

or advice would be greatly appreciated.

A few options to consider:

1. Jot down your own assessments of his progress (after all, you know

your child best) and then discuss these with the medical professionals

before they write up their assessments.

2. Have the physiatrist and/or other professionals evaluate his

progress in light of what you also have observed.

3. Consider having the SPECT scans reviewed for a second opinion.

Even if the SPECT scans show no difference, if the treatment continues

to be necessary to correct or ameliorate a medical condition, in the

judgment of a medical professional, then a case can be made for

continued treatment. As with the initial approval, a solid

recommendation from your medical professional would go a long way to

support approval of another round of treatment. It would be helpful if

the treating physician would not only write a prescription for further

treatments, but also support his prescription with a letter setting

forth what he has observed in terms of improvement and what conditions

he believes would be ameliorated by continued treatment. If the insurer

denies further treatments, then you will still have support for your

It does depend on who reads them, Sometimes the scan can look worse because

the HBOT is concentrating on other areas,,, Meaning the once bright part now

looks duller compared to another part that is now being healing with new

blood flow... Believe me I have seen it happen. Also the TIME after

treatment is a HUGE factor.....

I believe that you should wait until about a week or two to do the second

scan, because the most significant improvements are often seen AFTER the

treatment is over.

Multiple factors-including the efficacy of the antiepileptic drug for the

particular seizure type, availability of pediatrics-friendly formulations,

the ease of dosing and titration, and tolerability issues, such as possible

drug interactions and adverse events-affect the selection of the best

antiepileptic drug for a child with epilepsy. Behavioral problems are common

in children with epilepsy and can be aggravated or initiated by

antiepileptic drug therapy. The types and frequencies of antiepileptic

drug-associated behavioral events can influence a clinician's drug

selection, drug management, and counseling of parents. Unfortunately,

appreciating differences among antiepileptic drugs in behavioral adverse

event profiles is problematic because, among other reasons, methodologies

for reporting, collecting, and analyzing adverse events are not uniform

across trials, and there is marked heterogeneity in study design among

trials. This review summarizes incidence rates for behavioral and

psychiatric adverse events taken from studies of children with epilepsy.

These rates are reported for the 10 most commonly prescribed antiepileptic

drugs (valproic acid, carbamazepine, phenobarbital, lamotrigine, phenytoin,

levetiracetam, oxcarbazepine, topiramate, zonisamide, and gabapentin),

grouped according to their predominant mechanism of action. Despite the

numerous methodologic inconsistencies, some similarities in adverse event

profiles among antiepileptic drugs that share mechanisms of action are

apparent. Moreover, the overwhelming body of data on the behavioral effects

of phenobarbital should convince clinicians that, whenever possible, it

should not be used as the initial-or even the second-monotherapy in children

with epilepsy
