Make sure that you tell them that your household is of one person and that you do not work. Here are details on the program. Before you apply, please tell your doctor to write you prescription for 10 grams a day, since your prior history shows that you will not respond to 5 grams a day anyway. Testim only gets absorbed 23% better than Androgel (not double like your doctor may be thinking). You will need to send the Testim patient assistance people your prescription (or have your doc send it). You should not have to pay to see the doc again! A phone call should be enough. The patient must also reside in the US . Patients must be male. Patients who were eligible for Medicare Part D, but did not enroll may still be eligible for this program. Patients who enrolled in Medicare Part D, but are in the Donut Hole are not eligible. Applicants that do not qualify under the program’s set criteria may still apply by completing the “Hardship Exception Request†section of the application form. Application Process Anyone requesting assistance can call to request a faxed application or download it from the website. The application will be faxed out. The completed application must be mailed back. The patient is notified of eligibility for the program. The decision is made at meetings that occur approximately every month. (Speak to the company for specifics.) The medication is shipped within 4-6 weeks of receiving the application. Application Requirements The doctor must fill out a section, sign the application and attach a prescription for 6 months. The patient must fill out a section, sign the application and attach a denial letter from Medicaid. Program Details A six-month supply is sent to the doctor's office. The company automatically sends out refills. Every six months a new application is needed with a new prescription, once a year an application with documentation is needed. Option 2: Get a prescription for a compounded testosterone gel (5 percent testosterone works great for me). They usually cost $30 a month compared to the outrageous cost of Testim and Androgel of close to $700 a month for 10 grams a day. The prescription is written like this: Testosterone gel/cream 5 percent 30 mL in Topi-Click, 1 mL a day. I find the Topi-Click container to be a very easy way to administer the daily dose. You can also order the gel in a large graduated plastic syringe if you want to precise on your dose). I use APSMEDS.com since they have the cheapest price. For blood work, some people with no insurance monitor themselves by using labs that do not require a doctor's prescription: http://www.privatemdlabs.com/index.php You can see their tests and costs in that web site. Input your zip code and it will tell you the labs in your area. You can then post the results on this list and we can help you out, Estradiol and free/total testostosterone, and a complete blood count with differential (CBC) is all you need to monitor yourself after the first two weeks and then every three months.
