I am wanting to compile a sheet for my appeal letter so that CIGNA can see that this is being covered not only by them but by other companies. We have Cigna PPO, Executive Plan coverage (in San Diego, CA); they covered

100% ($3000) for the DOC Band. We are waiting for our 2nd band approval

now (Jaxen is outgrowing his 1st band).

Just wondering if you used the following information provided to me from the

Chicago Cranial Tech. office... It was photocopied and looked a little

ragged, so I just Typed it up and highlighted the insurance company we were

fighting with (which, of course, happens to have been CIGNA....) I also

site this information (that Cigna has paid before) in our letter.

Approximately 95% of all insurance carriers approve and cover the cost of

the DOC Band Treatment. The following is a partial list of a few of the

insurance carriers that have approved and paid for the DOC Band Treatment.

Kaiser, Unicare, Texas Medicade, Blue Cross Blue Shield of AZ and TX and CA,

Cigna, American Community, State AHCCS of Arizona, Humana, Aetna, Health

Partners Plan, Intergroup, American Family Insurance Group, Corporate Health

Administrators, Champus and Partners Health Plan, and Gem Insurance, Anthem

Blue of Indiana, Tri Care, Prudential, First Health, Motorola, FHP, EDS, and

Principal.

No...actually I didn't see that one...thank you...I am going to be including

that into my letter! I have cited that Cigna has paid before...but so far

Domi,

I had Capitol District Physician's Health Plan,

They denied coverage the first time, and approved 80%

coverage with only 1 appeal. We live in New York.
