Eme is on a continuous night feed. Our gi doc told us that the only fda

approved enzyme in bead form approved to go down the tube is pancrecarb.

With many weeks of practice (definitely not standard technique to get it to

wor we could finally easily get these down the tube and then had to work on

increasing to get the right dose. For us we actually found that Eme did

better on the viocase, but we were waking every 3-4 hours and shooting some

down the tube, I wondered about placing it in the formula so we wouldn't

need to do this. She is taking food orally now during the day, but not

enough calories to keep her weight where it should be. How much viocase

were you giving? How long was it hanging in the formula at a time?

We give Eme 1/2 tsp. at the begining of feed then every four hours after

that. She is getting 90ml/hr of 25 cal pregestimil formula. Seems to work

well for her. We mix the viocase with about 10ml of tap water. I also add

her reglan with it to keep her from getting over full. Seems to work for

her, but I would like to try the pre mixed in the bag of formula.

I give her 85 mg of Viacase per 100 cc's of formula. I put in the

whole nights feed at once with the enzyme mixed in. This worked for

us several months ago when she was on full feeds. Then she got

switched to TPN and we are now slowly trying to get her back onto her

g-tube feeds and she seems to be having problems with it. She seems

really uncomfortable and cries a lot. Her stools are getting greasy

and smell pretty bad. It is just strange to me that the same method

that worked before isn't working now, but it does seem to be an

enzyme problem to me.

How much Viacase do you give? When you give it a few times a night,

what do you mix it with?

HI

When my son was on TPN continuily and 10 hour nite G-tube feeds, we had the same

problems with the enzymes. Your not supposed to add them to the feed, I was

told.

So what we ended up doing was give the enzymes every couple of hours. This

became quite a hassel because I would wake-up mix the enzymes and put them into

my sound sleep baby. Which most of the time woke him up.

So after a few months of this I begged for another solution.

We started giving the enzymes 1 hour after the feeds began, 1/2 way thru and

then at the end. This was the solution that worked for us.

It's so tough having to do the TPN and the the g-tube feeds.

I wish you luck in finding a solution, I hope mine helps.

My daughter is 1 year old and gets continuous g-tube feeds and TPN.

My question to those that do continuous feeds (maybe even just night

feeds) is how do you handle enzymes. She is on Viacase and we have

been just mixing it into her feed, but lately she seems to be having

trouble with digesting again and I was wondering if there was a

better way to do this. She won't take any oral feeds yet.

Don't you know there are no stupid questions here.

TPN..total parental nutrition. what is that, right? lol I said the same thing.

It's IV nutrition for people who cant take food orally or have problems gaining,

GI problems ect...The IV bag is full of vitamins, water, sugar salt..all the

electrolyte balances and any meds they can add all go directly into a vein. You

also have to have a Picc line or a central line to receive the TPN, peripherals

arent big enough to handle the volume.

Hi,

I was just wondering for those of you whose kids have to be via a tube, when

this started? Was it when they were born/very young or when they hadn't been

thriving for a while?

started tube feedings within a few weeks of birth while in NICU

because she just couldn't take enough food. At the time, she was

taking about half of her feeds with a bottle. Then she had to go 1.5

months without feeds because of an intestinal blockage that needed

surgery. At first, she was too malnourished to handle the surgery so

they fattened her up with TPN, then she came down with full blown

psuedomonas pnueomonia and was too sick for the surgery. By the time

she was able to eat again, she had no interest. She was very orally

defensive because of all the tubes that had been stuck down her

throat. Since then we have been working on getting her to eat, but

it has been a very slow process because she is sick so often and we

don't push her when she isn't well. Right now, the big push is just

to get her tube feeds up high enough to get her off TPN again.

Kris, there's no such thing in the CF world as a stupid question. My

daughter's 19years old and I ask questions about CF every day. And, by the

way, I have no idea what TPN is.
