interesting changes. Amiodrone is now considered a class IIb and is recommended

as a FIRST line antiarrhythmic instead of Lidocaine in shock refractory V-Fib

and pulsless V-Tach. It is also stated that Vasopressin may be more effective

pressor agent than Epinephrine for promoting the return of spontaneous

circulation. The algorhythm for cardiac arrest has Epi. 1mg q every 3-5 minutes

OR Vasopressin 40 U IV single dose. ( I don't know if Epi can or should be given

after the dose of Vassopressin if circulation does not return) Studies have

shown that cardiac arrest survivors who received high dose Epi. have more post

resuscitation complications then those who received the standard dose therefore

high dose Epi. is no longer recommended do to the potential harm. In

Monomorphic V-Tach with normal cardiac function FIRST line is now Procainamide

or Sotalol Amiodrone and Lidocaine are exceptable but not preferred. If cardiac

function is impaired Amidrone 150mg IV bolus or Lidocaine .5 to .75mg/kg. I

would like to here some feed back on these changes this is the most dramatic

changes I have seen to ACLS in quit awhile and will most definitely affect EMS

care. The initiation of Amiodrone to the ACLS guidelines will effect quit a few

services just do to the price of it alone.

I hope to recall this accurately.

New AHA ECC guidelines from Channing-Bete 1- are $20 on

hard copy or disk...

Paper delivery is prompt, disks will be out in mid-September.

AHA says their product will be out in a month or so and cost $24 for

paper...

Or the August 22nd edition of Circulation, but AHA won't even sell

it to you.

They do have it online and in PDF format including the algorithms

(jpeg) if you know someone with the special handshake to their site!

Yes Tim, there are details about bi-phasic defib... but I haven't

read it yet. I am still trying to digest the " Class Indeterminate " .

A little word to the wise...

Do not put all your faith in the new ACLS guidelines. The ACLS Manual will be

published in December. It is not at all uncommon for the ACLS Guidelines as

published in " Circulation " to change or deviate when the ACLS Manual is

published. They have done this each year and sometimes the changes are

significant. I would not change protocols or practice patterns until I see the

ACLS manual in December. Otherwise, your personnel may have to re-learn

algorithms and such when the ACLS Manual is published.

AEMT will be holding classes for AHA instructors to help all who need the

updates for BLS, PALS, and ACLS. In San call 805-9102. Outside S.A.

call and ask for at ext. 211 for details.

Where can we get a copy of the new ACLS guidelines? Searching the AHA home

page didn't do any good.

Thanks for the help.
