Correct use of CPT codes 99201 to 99205

What is the correct use of CPT E & M codes 99201 to 99205 for billing in a

private outpatient PT clinic? For initial evaluation I use 97001 for all

patients except for Workers Comp pts, (In Texas we must use 99201 to 99205

for evals.) I know I must use 97001 for Medicare. It has been recommended

to me, by a consultant, to use these CPT codes more often, not just for

workers comp pts. It has also been suggested to bill them at the same time

as 97001. I have a CPT code book and have read the definitions, but I am

not sure when/if I should bill these CPT codes for an initial evaluation. I

am in a private outpatient clinic in Houston, TX.
