Rhea,

I will try to attach a slideshow on examination and treatment of the m.

iliopsoas (m. psoas major and m. iliacus). It sounds like you are using many of

the right modalities. It is important that the the therapeutic laser be

directed exactly at the pain if you are using a IIIb. If you have a class IV it

becomes much more difficult to address m. psoas major problems due to anatomical

issues and depth of penetration of the laser. I have also learned that these

dogs tend to re-injure this muscle easily so controlled exercise is very

important. All attempts should be made to avoid explosive activities for

sometime following the resolution of pain. Pharmaceutical management can

include gabapentin 5-10mg/kg q12h and amitriptyline 0.25-0.5mg/kg q12h.
