Let her switch to lactose-free milk as a first step and see what happens - maybe

he drinks alot of milk...

2 Year Old with Loose BM's

Hi everyone......

My question is regarding my best friends two year old son. He is happy,

healthy, eating well and growing (they both visited this past weekend), but is

also having 4-5 loose stools daily. He has never had a formed BM, and he screams

during diaper changes. She is very frustrated with this, and her pediatrician is

not concerned due to proper weight gain. She has " decided " that milk is the

culprit, although I told her that it could be many other things. My question:

Would you be concerned?? Should I put a 2 year old on an elimination diet?? She

wants me to guide her on what to do, what tests to ask for from the Doc, etc.

(Both of my children had formed BM's by this age so I could not give first-hand

commentary.)

She does not live near me, so if you have expertise and live in the Boston

area (northern suburbs, preferably) and could see her and her son, please let me

know. Any advice would be greatly appreciated.

No. I had explored that, too. She even dilutes the juices he consumes. I

would guess he only drinks 4-6 oz of actual juice per day, all diluted to about

half/half. Does this sound like too much to you?

She had tried to take him off of all dairy.....for 3 days...and no difference in

the stool. Maybe this was not long enough to see a difference?? He loves

yogurt, cheese, and cottage cheese, too, so maybe he is getting too much. He

also eats a lot of soy (milk and edaname).....could that be it? I feel like I

am " just taking a stab a it " rather than having more concrete answers.

I would try eliminating dairy for 2 weeks and if that doesn't make a difference

add it back and take him off of soy for 2 weeks. If there is still no

difference I would have him tested for celiac disease (serum tTG and serum EMA)

preferably by a pediatric gastroenterologist. I am also wondering if he is

drinking a lot of juice, which could cause loose stools as well.

I would have her try to eliminate milk products for maybe a few weeks. It

could be soy instead, so she could try that next or at the same time. My son

just had a RAST test completed. My pediatrician had suggested it about 3 years

ago (he's now 5) and it can test for the major allergens. It is 2 vials of

blood and it took all of 1 minute to complete. I wish I had done it sooner. I

know that they requested milk and peanut butter be tested in addition to all of

the environmental allergens, but it is not like the skin prick test where the

child would have to endure multiple pricks. It may be a start for mom. I am

not sure what allergens can be tested, but the peditrician could shed some

better light on that part.

Also, possibly have her keep a food log to get a better idea of the daily foods

that he takes in to see if thee are any other possible foods.

I would get a second opinion from a pediatrician.

We also have a good chapter on this topic in our book, Pediatric Nutrition

in Chronic Diseases and Developmental Disorders, Oxford University Press.

Maybe she can get it at her medical library.
