Scrotal Varicose Veins - significance?

Saw a gentleman today with scrotal varicose veins.

 

Not angiokeratomas.

Not varicoceles.

Literally, superficial varicose veins of the scrotum - both sides.No inguinal hernias palpable.

 

I spoke w/ the local urologist about whether that was pathonomonic for anything -- ie pelvic mass, etc.

 

He wasn't aware of any concern -- although a right sided varicocele could be a concern for renal cell cancer - he didn't think actual varicose veins of the scrotum was significant.

 

It's difficult for me to figure out what source the superficial scrotal veins are from - see below.

 

Any thoughts on potential pathology that would cause the varicosities of the scrotal surface?
