2 EGHP's and Medicare (due to ESRD)

In the scenario where a patient has an EGHP of her own and also an EGHP from a spouse and Medicare(ESRD), does the second EGHP have to follow the 30 month COB or would Medicare be secondary to the first EGHP with the spouses EGHP being tertiary?
