Hi everyone,

Just curious how the individual programs handle the cost of outside

films to be re-read by your hospital's radiologist for pre patients?

If the radiologist use a cpt code not payable by Medicare (76140). How

would you handle in paying this charge? Would you pay the billed charge

or would try to negotiate to lower your cost?

Bill, if you can comment on this, I would appreciate it.

The donor had an abnormal mammo and was sent to the breast specialist

for clearance. It's is the breast specialist policy to have the films

re-read by a local Radiologist.

This should be part of the eval, correct? So, if the reading of the

outside films are not payable by Medicare, can't we negoitiate on a fee

and still charge on the cost report?

HI ,

Based on what you have described, the

professional fee can be paid by the hospital at the Medicare Rate. Most of the

time, when Medicare says something is not covered it still has a fee set.

Check that first and then negotiate the fee. Also, ask why the professional

fee code was one that was not payable by Medicare. I have a feeling that it

was coded that way because it was a re-read.
