The patient returns today for regular care promising to get in to see Dr. Saboe. He is seeing the psychologist in the clinic and while the doctor was finishing up a call, reclined on the couch. He felt the dizzy spell come on. He later came into my treatment room and when he reclined on the table he experienced the dizzy spell.

He then related that it may be related to the fact that he had not eaten all day and had recently eaten a snack. Hypoglycemia aggravated vertigo?

I got out my portable badger and forced it on him; badgering him to go see Vern.

I am sending a case to Dr. Saboe.

Let’s see if he is the big shot, black belt, all-knowing, nerve doc that he says he is.

The patient was in a motor vehicle accident resulting in moderate whiplash symptoms. He denies headaches, loss of consciousness, or difficult neurological symptoms associated with concussion. During the exam, the seated patient was directed into cervical extension. He threw his hands out to the sides and corrected his position as if he had lost his balance. He didn’t exactly feel dizzy but felt like he was losing his balance.

He said that he first noticed this when he was returning to the gym after the accident to lift weights. He was planning to do a left where he reclined in a supine position and reached overhead to hold the weights superior to his head. When he slid himself headward, his head sort of dragged on the bench for forcing him into extension.

I have tried to duplicate his motion and it appears that he only experiences this dizziness when he moves his neck into extension with his eyes directed up word. If he extends his neck and keeps his eyes looking inferior, he seems to be okay. These symptoms appear to be fairly durable as they have not improved over three or four weeks but are not worsening.

(Of course, I have tried the Kaiser treatment for positional neurological disturbances by instructing the patient to avoid those positions in the future but the patient insists that there is more recovery to be had. Spoiled American.)

Interesting challenge.
