They have a right to know what they have, what the prognosis is, what the proposed treatment is, what the alternatives are, and what the possible side effects are prior to any treatment. Indeed, a patient has a right to say no, even if by refusing treatment they might die. I as a medical professional cannot overrule their decisions. Here we are talking about forcing a person to undergo mandatory drug therapy (vaccination), when they have no disease, under the presumption that they might get a disease based on future poor behavior. This is medically unethical.....Questions remain. As the [HPV] vaccine may not be effective in the long term, will booster shots at $120 be required? What type of world is it when a large company basically can forcibly take money from our pockets? " - ph Desoto, Ph.D., ton Daily Mail Barbara Loe Fisher Commentary: Congratulations to two brave Americans, who have spoken up publicly for the informed consent ethic, which includes the right of parents to make informed, voluntary vaccination choices for their children. Each undoubtedly knew how much criticism and pressure might come as a consequence, but must have chosen to speak out, nevertheless, as a matter of conscience. They and all those who stand up for truth and freedom, despite the personal or professional risk, are following in the footsteps of those who came to America to escape persecution for their beliefs and fought to create a democracy that protects minorities from exploitation by the powerful. As we are all witnessing, proposed HPV vaccine mandates are rolling through many state legislatures at warp speed. State and federal health officials (who are supposed to be legally prohibited from playing politics) and drug company lobbyists are in state Capitols pressuring politicians to make every little girl in America get injected with three doses of HPV vaccine before becoming sexually active or be denied the right to attend school. They are voting to mandate a vaccine for a disease which cannot be transmitted in school and which will cost the parents, the states and the nation billions to prevent a cancer, which can be almost 100 percent prevented through either abstinence, condom use or annual pap smears. Citizens in every state, who want to be free to make vaccine choices in the future, should pick up the phone, get to a fax machine, send an email or get in the car and go visit their state representatives and senators and make their voices heard. There is only one mechanism in a democracy for getting vaccine laws passed, which support the right to voluntary, informed consent: vote for politicians who will vote for freedom or vote them out in the next election. Nor am I for cervical cancer, which is caused by the sexually transmitted HPV. However, as my previous column on this subject indicated, I am opposed to the government mandating the vaccine. I say leave the role of strong-arm drug pusher to the thugs on the street. There is no reason that a voluntary program, based on an informed and sensitive health education campaign, would not work. New Hampshire, consistently one of the nation's healthiest states, has a voluntary HPV vaccine program, with plans to vaccinate 63,000 girls ages 11 to 18 over the next four years. But wait, you say: Nearly 12,000 land students could not go to school Monday because they failed to get their required vaccinations for chickenpox and hepatitis. If a mandatory program doesn't work, what chances does a voluntary plan have? Here's the deal: Running a government-funded, voluntary immunization program doesn't mean doing nothing. It means getting serious about providing health education and access to health care. " Education is something that we do -- in the schools and, most importantly, in our communities and in the pediatrician' s office, " said Greg , a spokesman for the New Hampshire Department of Health and Human Services. " How do we do it? Old-fashioned legwork. We make sure the right people have the right information. We have people who go out at the regional and local level, sit down and talk to health-care providers and ask, 'What can we do for you?' Cervical cancer is a huge issue for us, and now we have this tremendous opportunity to educate and protect women even more. " The New Hampshire approach encourages residents to take more responsibility for their lives. With the government acting as partner -- instead of some antebellum massa -- parents are encouraged to make choices that are in the best interest of their children. And guess what? They usually do. So far this month, about 3,300 doses of the HPV vaccine have been made available to girls in New Hampshire. State health officials say they are receiving feedback and determining where demand is greatest and how soon to order more vaccine. " We are at the beginning of the process, but from the anecdotal evidence, there appears to be a great demand from parents, " said. All child vaccinations in New Hampshire are voluntary. The state doesn't kick girls out of school because they didn't get a vaccine. It understands that parents can become overwhelmed and need encouragement -- not just threats and kicks in the butt. And as a result, the state has one of the highest rates of child immunization in the nation. " We know that there are parents who have expressed concern about children and childhood vaccines, " said. " Our program was designed with an emphasis on education and addressing whatever concerns parents might have. The District does have more poverty, but we also have more money to deal with it. D.C. public schools get more than $1 billion a year to educate roughly 76,000 students. Use those resources wisely. With all of the teaching hospitals in this region, surely somebody can figure out a way to set up a realistic and relevant health-education curriculum in our schools. So are New Hampshire residents somehow smarter and better able to develop effective public health programs? Are they more concerned about their children than the rest of us? Hardly. There are over 100 types of HPV, of which 35 are known to be acquired sexually. Infections with HPV subtypes 6, 11, 16, 18, 34, 36, 45 are especially prone to cause cancer. Recently, a vaccine developed by Merck was shown to be effective against HPV subtypes 16 and 18, which together cause 70 percent of today's cervical cancers. This news prompted many of our state legislators to run to the microphone and push for the mandatory vaccination of our 9-year-old daughters. Were they acting in our best interests or pushing an agenda? The HPV virus can be passed through direct, skin-to-skin contact or through contact with the bodily fluids of a person who is shedding the virus. In terms of the sexually transmitted form of the virus, a person is likely to either be infected or exposed to more than one subtype of the HPV virus. The vaccine, however, suppresses only a few specific HPV types. Hence, those subtypes not suppressed will obtain an evolutionary advantage in growth and become more prevalent and dominant -- for which the vaccine will have no effect. This is the analogous reason we don't have a generalized vaccine against the common cold, though we do have vaccines against some individual cold viruses. Additionally, according to the data presented to the FDA, which I have reviewed, it was evident that 1) there is no evidence that this vaccine works after five years, 2) two-thirds of those who had been administered the vaccine suffered from moderate to severe pain at the site of injection, 3) it is not known whether in the long term this vaccine may cause auto-immune, neurological or other disorders, 4) the risk for pelvic inflammatory disease, appendicitis and gastroenteritis is doubled. This is medically unethical. We do have as public policy mandatory vaccinations for some infectious disorders such as mumps, measles and rubella. This is true in part because of the idea of herd immunity -- that is, if 97 percent to 98 percent of a population is immunized against a disease, the disease may be nearly eradicated. That argument doesn't hold in the case of the HPV vaccine, as it is an incomplete vaccine. We also forget that men make up half of those infected with HPV. Thus, herd immunity will not develop. In order to understand the present, sometimes we must look into the past. It was the Merck Co. that produced the COX-2 inhibitors as an alternative to aspirin-type drugs (non- steroidal anti-inflammatory drugs) under the idea that these COX-2 inhibitors would have fewer side effects. Merck duly sponsored clinical trials, which did show that COX-2 inhibitors were superior to NSAIDs in terms of reducing side effects -- or so we thought. This wasn't really true, as we know that COX-2 inhibitors actually increase the risk of heart attacks. Studies showing this increased risk of heart attack were suppressed from the medical journals prior to FDA approval for the drug. The Merck vaccine cost $120 a dose, and three are needed, for a total cost of $360 a child. Multiply this times every school-age child in the country. This means billions. Questions remain. As the vaccine may not be effective in the long term, will booster shots at $120 be required? What type of world is it when a large company basically can forcibly take money from our pockets? Why are some legislators really pushing this vaccine? I do believe the HPV vaccine has some value. However, the idea of mandatory vaccination should be approached carefully. DeSoto, who has a doctorate in pharmacology, is a fellow in the American Institute of Chemists.
